How ID proofing works?
To schedule an identity proofing appointment please select/click your EMR / EHR service provider logo above and follow the steps. You will receive a secure encrypted document link. Complete Section 1 of the identity information application and include your National Provider Identifier (NPI). Also input your DEA registration number (If applying for EPCS). Please electronically sign and date. Click submit.
You will receive a direct phone call or an invite to join our Agent in audio/video conference.
Please be prepared to display your Identification document(s) to our Agent for verification of your identity. You will receive the completed identification information application document.pdf link for you to download and retain for your records. Your completed application will be forwarded to your EMR/EHR to complete your registration.
